What is the All-on-4® Treatment Concept?


When you have lost an entire arch of teeth, a full arch restoration is used. Typically, this treatment involves the use of 6 to 8 implant posts. However, for implants to be successful, you need to have sufficient bone mass in your jaw.

Without enough bone mass, your jawbone cannot properly fuse to the posts. This can result in loose implants and implant failure. Even the failure of a single post can mean the failure of the entire restoration.

In many cases, a bone graft is required to restore strength to your jawbone. This is an additional surgical procedure with additional healing time. The All on 4 treatment concept provides a full arch restoration without the need for a bone graft.



What are Dental Implants?


A dental implant consists of a post, abutment, and restoration to replace a missing tooth.Dental implants are a modern treatment used to replace missing teeth. This treatment is unique from traditional treatments in many ways.

One of the biggest differences between traditional treatments and dental implants is that implants act as a complete replacement tooth. They require a surgical procedure, in which small posts are surgically placed into your jawbone, and they can restore a single missing tooth or multiple missing teeth.

As you heal from surgery, your bone gradually fuses with these posts in a process called osseointegration. This process stabilizes the posts within your jaw, essentially turning them into the roots that will then provide support for your replacement teeth. Implants are a versatile treatment and can replace a single tooth to an entire arch.



Why Should I Replace Multiple Missing Teeth?

How does tooth loss impact the jawbone?


When you are missing teeth, your other teeth will begin to 'collapse' in around the missing areas and your jawbone will begin to lose bone mass- but dental implants can stop this from happening.
1.  The teeth provide stimulation for your jaw. Every time you chew, this stimulation tells your body to send essential nutrients. Following tooth loss, however, simulation is lost.

2.  Your body begins to think the jaw needs fewer nutrients. Since it thinks the jaw doesn't need nutrients, it therefore does not send as many. Over time, your jawbone gradually begins to weaken. This alters the shape of your jawbone, which can then also alter the shape of your face.

3.  With dental implants, however, we can stop and prevent bone loss in your jaw. The posts function just like the roots of your natural teeth. In addition to restoring your abilities to eat and speak and restoring the quality of your smile, dental implants help to restore stimulation to your jawbone, once again letting your body know that nutrients are required.



How Does the All-on-4® Treatment Concept Work?


The All on 4® Treatment Concept is not unlike any other type of dental implants, but it is specially designed for those with low amounts of jawbone to anchor the implants into. The success of this treatment is not so much the number of implant posts, but how these posts are placed within your jaw.

The All on 4 Treatment Concept involves 4 dental implants placed at a strategic angle for patients with low amount of jaw bone.All on 4 uses only 4 posts. This treatment involves the surgical placement of implant posts into your jaw to provide support for a full arch of replacement teeth.
•  Two of these posts are placed vertically at the front of the jaw, where the bone is naturally thicker and stronger.
•  The other two posts are placed at the back of the jaw, inserted at a 45-degree angle.

At this angle, the posts are providing optimal contact with your jawbone, maximizing osseointegration. Your bone can properly fuse to the implant posts and turn them into the stable roots that your replacement teeth need. Because of this, the need for a bone graft is eliminated.



How is All on 4 Different from Other Dental Implant Solutions?


The major difference between All on 4 dental implants and other multiple dental implant solutions is the number of implant posts that are used. With most full arch restorations, we typically use 6 to 8 implant posts.

There are other treatments, such as bar retained dental implants, that only use 4 posts, but your teeth clip into place on a titanium bar that is attached to your implant posts. The teeth are still removable and need to be removed for cleaning. All on 4, your teeth are screwed into place on your implant posts just like other types of dental implants. This prevents the need to remove your teeth every night to clean them. You can continue to care for you're All on 4 implants just like your natural teeth.



Does My Insurance Cover All on 4?


More and more insurance companies are beginning to provide coverage for dental implants, as well as All on 4. However, most that do provide coverage only provide a portion. This is because, while dental implants provide health benefits, they are still seen as a cosmetic procedure. You will need to contact your dental insurance provider to find out if they do indeed cover All on 4, and if so, how much they will cover.



How Much Does All on 4 Cost?


The cost of All on 4 depends on many factors. These factors include scans and X-rays, extractions, necessary dental procedures before treatment, the materials used, anesthesia costs, and the number of office visits needed. Typically, a single arch can range anywhere from $15,000 up to close to $35,000 before any type of insurance coverage is applied. This treatment costs less than a regular full arch replacement that uses 6 to 8 implants. It also costs much less than undergoing a bone graft and then receiving dental implants.



What is the Process to get the All on 4® Treatment Concept?


The process for All on 4 requires a surgical procedure that is done under local anesthesia and sedation, a healing period, and the attachment of a final restoration.
1.  Consultation: During your initial consultation, we will go over your sedation costs to determine the best one for your needs.
2.  Surgery: Surgery begins with small incisions in your gums to expose your jawbone. Holes are drilled into your jaw based on your treatment plan. The posts are placed immediately inside, and your gums are sutured closed. Before you leave the office, temporary teeth are affixed to your implants, which are used until you heal.
3.  Healing: We wait until you have fully healed from surgery, a process that can take several weeks to several months.
4.  Restoration: We then design and create your custom dental restoration and secure it into place on your implant posts.



An implant-retained denture is an alternative solution to the All on 4® Treatment Concept.

What are Alternatives to All on 4?


There are other alternatives to the All on 4. These options include:
•  Traditional Dentures: These are a removable prosthesis that sits directly on your gums.
•  Implant-Retained Dentures: Implants provide stability for your denture, but your gums still absorb a majority of your chewing forces.
•  Bar Retained Implants: 4 implant posts are placed in your jaw and are connected by a titanium bar. Your replacement teeth clip into place on the bar and still need to be removed daily for cleaning.
